Performance evaluations due
(May 14, 2008) All supervisors of faculty, exempt professional and classifed staff are reminded that performance evaluations for the 2007-2008 cycle are now due in the HR office.

Performance Management Documents
The final overall rating for each faculty member and exempt professional staff member is recorded on an Annual Performance Rating Form, also known as the Public Rating Form (http://www.administration.ucdenver.edu/admin/hr/documents/OPENRECORDSFORM-FACULTYEXEMPTEVALUATON.doc) and is submitted to the Human Resources office. Ratings for entire units located at the Anschutz Medical Campus or 9th Avenue may also be submitted to our office via spreadsheet.
Per long established Regent Policies, Fiscal Year 2008-09 salary adjustments will not be allowed if ratings are not submitted.

This is the form to use for developing goals and measurements for the period of April 1, 2008, through March 31, 2009. Please submit only the cover/signature page and the scoring matrix page containing the weights for each of the goals and competencies to the Human Resources Office (campus box A005/130, attn: plans).
Training Requirement for Supervisors
